This page is run by the clinic. UpcharHub provides the software.
This form collects your name, mobile number, preferred day and time, any note you send, and the time you sent it.
This page uses Google reCAPTCHA to stop spam. For that check, Google receives information about your device and internet connection, but not your name, number or note.
The clinic uses these details to contact you about your request and, if it accepts, to add you to its patient register.
The doctor, the clinic's front-desk staff, and UpcharHub can see these details. Google also receives the name and note for the automated check. Meta/WhatsApp receives the number and code for confirmation.
When the clinic adds you to its patient register, your details are saved to a patient chart at that clinic: your name, mobile number and note for a new chart, or only your note if the clinic already has a chart for your number. The clinic keeps that chart; it is not deleted with the request.
Once the clinic accepts or declines your request, the request is deleted within a few days. A request nobody has answered is deleted about 90 days after it arrives.
To have your request deleted sooner, ask the clinic to decline it, or email contact@upcharhub.in with your mobile number and the clinic's name.

We set a cookie on this browser for up to 180 days. It holds a random ID, not your number, and helps stop abuse. It lets us remember for 30 days that a number was confirmed on this browser.
An automated check using a Google model, served from the Mumbai region of Google Cloud, reads your name and note to stop abuse before the clinic sees them.
We send a code to your number on WhatsApp (Meta) to confirm it is yours. The clinic does not see a request until its number has been confirmed. A number confirmed on this browser in the last 30 days is not asked again. An unconfirmed request is deleted after 15 minutes, at the next clean-up.
